1. Proceed to the CSO Reception Desk to begin a transaction
with the CSO Desk Officer.


Submit all Accreditation supporting documents.

1.1 CSO Desk Officer will conduct preliminary review of the documents submitted.


1.2 Upon determination of orderliness of the submitted documents, the CSO Desk Officer will stamp “received” the Application Letter and Transmit the documents to Sangguniang Panlungsod for processing of Accreditation.


1.3 The CSO Desk Officer will advise the client to wait for the notification from
Sangguniang Panlungsod on the date of claiming the Accreditation Certificate.

1.4 If the submitted documents are incomplete or not in order, the CSO Desk Officer will return them to the client with corresponding notes and advise.
