1. Unified Application Form for Business Permit (1 original copy)  Business Permit and Licensing Office

3. Valid I.D. with signatures (2 photocopies)  Business Owner
4. DTI Business Name Registration for Sole Proprietor, SEC Registration for Corporation and Partnership, and CDA Registration for Cooperative (2 photocopies)

Note: for corporation, partnership, and cooperative, pls. attach articles of incorporation or articles of cooperative whichever is applicable (1 photocopy)

5. Barangay Business Clearance for the additional line of business (1 original copy, and 1 photocopy) Concerned barangay where the business establishment is situated 
6. Homeowners Association Clearance, if the business address is inside the subdivision - for the additional line of business (1 original copy & 1 photocopy) Homeowners Association
7. Market Clearance, if the business operation is inside the public market building/compound (1 original copy)  OCM-Operations of the Public Market
8. Business Permit, Sanitary Permit, FSIC, CENRO Clearance, Vet Clearance, Tourism Clearance (1 original copy each) issued in the current year Business Owner
9. Special Power of Attorney, if applicant is not connected to the owner of the business (1 original copy and 1 photocopy); and Business Owner
10. Valid Identification Card of the authorized representative; or Business Owner

1. File the application for the additional line of business

1.1 Shall submit all the documentary requirements and the accomplished Unified Application Form at Step 1 (Windows 3-5)

1.1 Shall check for the completeness of the documentary requirements submitted

1.2 Secure necessary clearances/ certifications/ permit from endorsing offices particularly Zoning Clearance, Building Official Clearance, Sanitary Permit, CENRO Clearance, Vet. Clearance (if the business is animal facility/clinic, animal farming, selling of veterinary products), Tourism Clearance (businesses related to tourism)


1.3 Encode and print the e-business applications form upon completeness of the clearances/certifications/permits from endorsing offices.


1.4 Forward the business application to Assessment of Billing at Windows 6 or 7

2. Receive the Order of Payment


2.1 Proofread the billing statement and ask question if there is any, if none receive and sign the billing statement, accomplished unified application form and documentary
requirements and clearances at Windows 6 or 7

2.1 Assess and print the Billing Statement in 4 copies.


2.2 Issue Tax Order of Payment.


2.3 Keep the receiving copy.

3. Pay & Claim at BPLO Windows 8 or 9


3.1 (Pay the amount indicated in the Tax Order of Payment, and BFP Order of Payment Slip)

3.2 Shall sign the Business Releasing Logbook


3.3 Receive the Official Receipts, updated Business Permit, Sanitary Permit, Fire Safety Inspection Certificate, CENRO Clearance, Vet. Clearance (if the business is animal facility/clinic, animal farming, selling of veterinary products), Tourism Clearance (businesses related to tourism), etc

3.1 Shall receive payment of taxes, fees and charges.

3.2 Shall process and print Fire Safety Inspection Certificate (FSIC)

3.3 Shall approve and print Business Permit Certificate 

3.4 Segregate the attached documentary requirements for Business tax payers and for office files.


3.5 Shall bring the Business Permit to the Local Chief Executive or authorized representative for approval and signature


3.6 Stamp a dry seal


3.7 Record in the releasing log book and have it signed by the client as proof of receipt.


3.8 Release the approved Business Permit, FSIC, Official Receipt, and other related clearances/ certificates, and Permits.
